What should I bring for my first appointment?
When you first come to our dentist in Decatur, we would like you to arrive 10 to 15 minutes early to complete new patient paperwork. Do not forget to bring a valid photo I.D. and your dental insurance card (if applicable). As part of learning all about you and your medical history, we would like you to tell the doctor about any medications you are currently taking.
